Care Assessment
Our services begin with a free care consultation and assessment, conducted
by a Nurse Care Manager located in the Corvallis area. Our care assessment
sets the foundation of your in-home care. Because each family is unique,
each home-care plan is also unique. During the care assessment, New
Horizons Nurse Care Managers will help craft a specialized plan for your
care. This visit is done free of charge with no obligation for future services.
The foundation of our caregiving philosophy is respect for the individual.
Our nurses are sensitive to the fact that every person is different. Their
reactions to the idea of in-home care is different, their needs are different,
and their living situations are unique. We take time to get to know each
person and the needs that they have, whether it’s around-the-clock care or a
few hours per week of housework assistance.

Personal Attention
The best in-home care comes from building foundational, personal
relationships with the families of those in need. At New Horizons, we know that deciding to use in-home care for your loved ones can be a tough and
emotional decision. Because of this, the initial care assessment allows both
your family and our local Corvallis New Horizons staff to get to know each
other on a personal level, finding the best solution for you.
Our Corvallis area nurses assess the disposition of the person, their abilities, and their needs through observations and conversation. In addition to a
physical exam, such as listening to lung function, our in-home nurse care
managers look at how easily the person is able to walk from one place to
another. Friendly conversation can reveal their thought processes, memory,
decision making, and judgment, as well as if they have difficulty expressing
themselves or understanding what other people say. Our care providers will
talk to the other people involved in their care, including family members
and other Oregon health care professionals, to understand if some difficulty
that the patient may be having is perhaps related to a stroke, or if there is
some other underlying cause such as vision problems.

Each care assessment is conducted by one of our Corvallis-based New Horizons’ Nurse Care Managers; all are experienced in long-term care. Whether you have a baby on a ventilator or just want help with housekeeping, our personal approach to in-home care is based on the individual needs of each person we care for.
Finding Comfort
Your family’s comfort in a trying time is very important to us. We’ll conduct
the care assessment wherever it’s most convenient for you; a meeting at your
home or any other location of your choosing. During this visit, we will
discuss what services you are interested in. We can also help evaluate your
home setting and make suggestions for safety and convenience.
Creating a Care Plan
Our Care Managers will discuss exactly how our services will work for
you, including our fee structure. We can also connect you with additional
community resources in the Corvallis, and Linn-Benton County area such
as delivery of meals, respite and day care options, relocation options, etc.
New Horizons will help you create a comprehensive plan of care that acts as
the road map to your future, considering:
